You have probably already discussed with Kythera Ann, by e- mail or phone, what you want, and agreed on an appropriate price for the services you need. If you have not, please click here to start a discussion and include in your e-mail any questions you may have, what your service needs are, and what your site is about. If you have already done the above, and Kythera Ann directed you to read the TOS and then follow the instructions on this page, read on! When you click the button below, an e-mail window will pop up with the subject line filled in. Don't change the subject line! It is our legal record that you have read and agree to the TOS. In the body of the e-mail, please provide the following information: Your full name (for formal correspondences) The name you would like to be called by at Elysium Gates (may be the same, may be a screen name) Your home address (where we can send you gifts, prizes, and holiday cards!) Your billing address (if different and you have arranged for invoiced payments with Hermes our accountant). Your home phone number (just in case) Any email addresses not associated with your domain (we want to always be able to contact you!). Your birthday, include year of birth. We won't tell the year, but how else can we celebrate? *wink* The URL of your site (you will need to send one of these forms for each site you host with us). OK, clear on everything now?
